Requirements In Securing bldg. Permit

Five(5) Sets of Construction Plan
    a. Archirtectural & Structural Plan and Application form         Signed & Sealed by Architect / Civil Engineer and            
        in-charge of construction
    b. Xerox of PTRC/PRC
    c. Sanitary / Plumbing Plan and Application Form Signed and         Sealed by Sanitary Engineer Master Plumber
    d. Electrical Pland and Application Form Signed and Sealed         by Profesional Electrical Engineer / in-charge of         construction
2. Five Copies of Locatin Plan with vicinity Map (Signed and     Sealed by Geodetic Engineer)
3. Specification (Signed and Sealed by Architect / Civil     Engineer)
4. Bill pf MAterials (Signed and Sealed by Architect / Civil     Engineer)
5. Structural Computation (For Two (2) Storey and above) with     Signed and Sealed of Civil / Structural Engineer
6. Soil Analysis (For three (3) storey and above)
7. Xerox Copy Title (Certified True Xerox Copy of Transfer     Certificate of Title(TCT)
8. Xerox Copy of Tax Declaration
9. Xerox Copy of Tax Receipt for the current year
10. Barangay Clearance
11. Homeowners Presidents Clearance / CRO Clearance, in       the absence of Homeowners       Association
12. Locational Clearance
13. Construction Logbook
14. Mayor's Permit of the Contractor
15. Construction Clearance from the Developers
16. Affidavit of Consent if Lot is being Leased / or from Lot       Owner (Notarized)
17. Written Authorization from the owner (in the absence of the       absence of the Property owner)

Business Permit Application Procedures

For New Applicants

  1. Applications must be received and approved by the Business Permits and Licensing Office (BPLO) before engaging in any business.
  2. Attach all required supporting documents then submit the same to BPLO.
  3. An acknowledgement receipt will be given to the applicant.
  4. The business establishment will be inspected.
  5. When everything is in order, assessment will be prepared and to be paid to the Treasury Office teller immediately.
  6. The business permit/license will be delivered to the place of business.
  7. The business permit/license must be displayed in any “easy to see” place of the establishment.
  8. The business permit/license is to be renewed on or before the 20th of January every year.

  2. SSS Clearance
  3. DTI/SEC registration
  4. Community Tax Certificate
  5. Photo of establishment (showing signboard & sidewalk)
  6. Waste Management Seminar Certificate
  7. Engineering Inspection Certificate
  8. Health Inspection Certificate
  9. Fire Inspection Certificate
Other Supporting Documents
  1. Homeowner’s Association Clearance – if in subdivision
  2. MSO Clearance – if located at settlement site
  3. Occupancy permit – for new & renovated structure
  4. Contract of Lease – for lessor and lessee
  5. Lessor’s permit – if rented
  6. Working permit – for workers in restaurants, beerhouses, carinderia, beauty parlor, food processing factory, etc.
  7. DECS Registration - for learning institutions
  8. DENR Certificate – for gravel and sand, pollutant company
  9. BFAD certificate – for drugstore or pharmacy
  10. Fertilizer & Pesticides Authority License – for selling such goods
  11. Park and Development Office – if located along riverbanks & parks
  12. Locational/Zoning Clearance
  13. Contract – for contractor, service contractor
  14. Market Clearance – for stallholders in public markets
  15. Potability Test Certificate – for drinking water vendor
  16. Traffic Clearance (PNP) - for transport terminal
  17. Main Contractor’s Permit – for sub-contracting company
  18. Philippine Shippers Bureau Certificate – for freight forwarders non-vessel operating common carriers, cargo consolidators breakbulk agent
  19. PNP firecrackers clearance – for vendor of such goods
  20. Dangerous Drugs Board Accreditation – for drug selling
  21. Trained Life Guards – for resort, hotel & other establishments operating swimming pools.
  22. Others (depending on nature of business)
For Renewal
  1. The Business Permits and Licensing Office must receive the renewal application on or before the 20th of January every year.
  2. Attach required supporting documents, attach them to the duly filled-up application form and submit it to BPLO.
  3. The applicant will be given an acknowledgement receipt.
  4. Order of Payment will be delivered to the place of business in which the amount indicated therein must be paid to the Treasury Office teller within seven (7) working days upon receipt of the same.
  5. The business permit/license will be delivered to the place of business.
  6. The business permit/license must be displayed in an “easy to see” place of the establishment.

  1. Barangay clearance
  2. Valid SSS Clearance
  3. Previous year’s business permit
  4. Health Inspection Certification
  5. Engineering Inspection Certificate
  6. Fire Inspection Certificate
  7. Occupational Permit – for workers in restaurants, beerhouses, carinderia, parlors, etc.
  8. Others (depending on nature of business)
For Retirement

The Business Permits and Licensing Office must receive the application within twenty (20) days. After operation of business is stopped/closed.
  1. Complete the required supporting documents, attach them to the duly filled-up application form and submit the same to BPLO.
  2. Applicant will be given an acknowledgement receipt.
  3. The business establishment will be inspected to see if the business has really stopped/closed permanently.
  4. In case there is still any amount to be paid, assessment will be done and must be paid to the Treasury Office Teller.
  5. When everything is in order, retirement will be approved.

  2. Official Receipt representing payment of Business Permit
  3. Community Tax Certificate
  4. Financial Statement
  5. Others
For Occupational Permits
  1. The application must be received and approved by the Business Permits and Licensing Office before engaging in occupation that is subject to inspection and surveillance.
  2. Complete the required supporting documents, attach them to the duly filled-up Application Form and submit the same to the said office.
  3. When everything is in order, the Occupational Permit will be approved.
  4. The Occupational Permit is to be renewed on or before the 20th of January every year.
